About this role
Home Health Occupational Therapist (OT)
Per Diem | San Francisco or San Mateo County — Dedicated Territory Assigned
Where great OTs come to do their best work.
At Health Link Home Health & Hospice, we believe great patient care starts with supporting the clinicians who deliver it. For over 20 years, we've served the greater Bay Area with a focus on clinical quality, strong systems, and a collaborative culture.
We are hiring Occupational Therapists for both our San Francisco and Peninsula teams. You will be assigned to one dedicated territory — SF County or San Mateo County — so you can build familiarity with your patients and your area without being stretched across both.
Why OTs Choose Health Link
Dedicated Territory: Your caseload stays within a manageable radius in either SF or the Peninsula — no crossing bridges or unpredictable commutes. SF patients are spread across neighborhoods like the Sunset, Richmond, and Mission; Peninsula patients run from Daly City down through Redwood City.
SF Rideshare Support: If you take the SF territory, company-provided rideshare credits (Uber/Lyft) help cover travel to and from patient visits so you can focus on care, not logistics.
AI-Assisted Documentation: We use AI-assisted charting so you spend less time on notes and more time with patients.
Clinical Support: You are never alone. Our experienced case managers and clinical team are available in real-time to assist with complex situations or patient coordination.
